We have a team of iCare approved case managers. We provide these services either short term or long term, pending on the participants needs and situation. Having a locally based case manager ensures that we are well connected with suppliers and providers in the community. We know what is available, we work hard to have good relationships with other providers and we find this leads to more efficient and better outcomes for our participants.
The aim of case management is to maximise independence, self efficacy and quality of life. We do this by partnering with the participant, their family, informal and formal supports, treating team and funding body. We stay well connected with the participant to ensure ease of communication and follow up occurs.
We also aim to have a good working relationship with the iCare coordinator, claims advisor or other funding body contact. We do this with prompt communication, advising of any changes for the participant or with case management services.
We enjoy the role of case manager and supporting the participant and team to work towards common goals. Problem solving through challenges, supporting participants to achieve goals and helping the team navigate the requirements are some of the things we enjoy.

- Psychosocial injuries
