NDIS

We provide Dietitian services for adults and children who are under the NDIS scheme, under either plan or self managed arrangements only. We provide Occupational Therapy services for anyone over the age of 16 . Please note we currently have a lengthy wait list for OT services. We have immediate availability for our dietitian services.

Our Founder

About Louise

I graduated as an Occupational Therapist from the University of Newcastle in 2005. Since this time I have worked in rural NSW in a variety of roles including mental health, community health and in-patient acute wards. 

Prior to becoming an OT I worked in a variety of roles with adults with disabilities. I have worked as an OT in the Gunnedah are for 10 years. I have a well established network with local providers and make it a high priority to work collaterality with the participant, their family and the team to ensure we can get the best outcomes for participants. I thoroughly enjoy working with participants and their families and problem solving to contribute to great outcomes. 

NDIS

Our Dietitian services include:

Nutritional support for symptom-based concerns- issues like Diarrhea, vomiting, constipation

Nutritional support for weight concerns - weight loss advise if overweight or underweight.
Fussy eating or restricted diets.

Nutritional enhancement menu planning.

Nutritional support for wound healing/management.

Nutritional support relating to chewing or swallowing issues.

Nutrition risks such as Malnutrition & weight loss. ‍
Enteral (tube) Feeding.                    

Chronic Disease Management - Diabetes management, blood glucose management, hypertension, respiratory concerns etc.

Our Occupational Therapy services include:

PLEASE NOTE: We currently have a lengthy wait list for NDIS OT services in all towns that we cover. The wait time varies pending on clinical risk and location.   • Minor and major home modifications

Home modifications include but are not limited to things like grab rails, ramps, changes to bathroom.

  • Equipment (Assistive Technology) prescription

This can include things like wheel chairs, shower seats, kitchen aids (built up cutlery).

  • Functional and Activities of Daily Living (ADL) assessment and report

This involves an assessment of what assistance, modification and equipment a person requires to achieve their goals in their daily life.

  • Supported Independent Living (SIL) assessment and report

We provide a comprehensive and wholistic assessment and report detailing what the participant requires to live their daily life, and submit to the NDIA for review.

  • Supported Disability Accommodation (SDA) assessment and report

We complete a comprehensive assessment and report considering housing needs related to the persons disability, and submit to the NDIA for consideration.

Work Experience

I have been working privately as a OT and Case manager with iCare, CTP and NDIS participant since 2011. I formed Focus on Function in 2021 as there were limited Occupational Therapy services available in the New England. I am committed to providing his quality services for our participants, families and community. 

I am a approved Case Manager with iCare (lifetime care and workers care). I am a member of Occupational Therapy Australia and a registered with the Australian Health Practitioner Regulation Agency.
