We provide Occupation Therapy, Case Management & Dietitian services for people with severe and permanent injuries under NDIS, iCare Lifetime Care and iCare Workers Care, and CTP claims. Case management includes coordinating and communicating our participant's needs to ensure they can achieve their goals.

We provide Dietitian services for adults and children who are under the NDIS scheme, under either plan or self managed arrangements only. We provide Occupational Therapy services for anyone over the age of 16 . Please note we currently have a lengthy wait list for OT services. We have immediate availability for our dietitian services.

We strive to provide collborative, evidence based, cleint centered services for complex request. We have icare approved case managers, FIM (Functional Independence Measure) accredited OTs, panel approved Care Needs Assessors and Accrediated Practicing Dietitians.

CTP Claims

We provide Occupational Therapy, Case Management and Dietitian services for adults with a variety of injuries under the CTP claim that will aid with recovery, achieve goals and improve daily living situations. This includes (but isn't limited to) initial needs and activities of daily living assessments, equipment prescription, home modifications, nutritional advise and case management.

Workers Compensation

We provide Occupational Therapy, Case Management and Dietitian services for adults with a variety of injuries under a workers compensation claim. This includes (but isn't limited to) initial needs and activities of daily living assessments, equipment prescription, home modifications, nutritional advise and case management. We don't provide return to work services. Our services are more focused on injury related needs to optimise participation, recovery and goals.

What Is Case Management?

Case Management involves assisting a person with a disability, injury or medical condition to access services they require to assist with management and recovery of their condition. The case manager is central to coordinating the recovery and rehabilitation related needs and goals.

Case Managers work with participants, their family, the local treating team, medical specialists and the funding body. This may include assistance with setting goals and support needed to achieve goals, coordinating therapeutic support, monitoring how the participant is progressing and assisting with adjusting the proposed plan as required.
